Komi faces a competition she has not agreed to enter during Itan High's health exam. Makeru Yadano turns routine measurements into a contest against the class idol, according to VIZ's synopsis. This is an early dose of the school comedy's appetite for disproportionate reactions. Choose it as the continuation of the opening social setup, with the joke resting on the distance between an imagined rivalry and an unaware opponent.

Makeru treats eye tests and height measurements as opportunities to defeat Komi. VIZ also makes the decisive comic qualification: Komi does not know a duel is happening. That imbalance is more interesting than the results of the tests. It puts the work of misunderstanding on a supposedly more communicative classmate, rather than making Komi's difficulty the only source of trouble. Even so, a routine health exam inflated into rivalry is not the reason to buy the run. It is a small comic application of the premise, with no need to dress it up as a major emotional advance.
