Ouran High School Host Club Volume 2 turns a routine school medical exam into a problem for Haruhi's place in the club. The setup depends on the difference between what the hosts know and what their customers believe. This is an early test of whether that comic premise works for you. It is not the reason to buy the run on its own: the appeal lies in watching a familiar arrangement struggle with ordinary school procedure.

The medical exam makes concealment a collective task. Haruhi's position depends on keeping her being a girl from the club's female customers, and the boys must work out how to manage a doctor who could expose it. Their concern includes the prospect of club life becoming boring without her. That is an affectionate motive with a selfish edge: preserving their amusement is part of preserving her job. The comedy comes from an institution outside their performance refusing to follow its rules. A medical appointment does not owe the club its preferred version of reality.
