Struggler is the name he gives Guts, and it is not a compliment so much as a job title. The 1997 series never has this scene, because it cut the Skull Knight out of the story altogether, which is why its final episode ends with Guts alive after the Eclipse and no explanation of how; a viewer of that version meets this line for the first time in the films or the 2016 series and discovers that the rescue had a price. Read that way the speech is less a rallying cry than a diagnosis delivered before the illness.
