“BUCCIARATI!!! I'm going with you! I'm right here! Don't leave me! Please don't tell me to stay behind! Trish and I are the same! Her wounds are my wounds! Her pain is my pain!”
The dub gives it the full exclamation-mark treatment and it still works, which tells you where the weight sits. Narancia has decided that Trish's pain is his, and he says so from the water, mid-stroke. The cost is announced in the same breath. Every member who boards the boat is on a road that ends in Rome, and Narancia's end is the one the story stages most cruelly, impaled on broken iron bars during erased time while his soul was in Giorno's body, and left behind with Giorno's promise to bring him home. Her wounds were his wounds. He took the sentence literally, and the series let him.

Bucciarati has just committed treason and given his crew the choice of following him or the organization, and Narancia has jumped into the lagoon to swim after the boat. The decision is out of proportion to what he knows. He has no idea who the Boss is, no plan, and a Stand that fights by aiming a toy plane; what he has is a history the series has already shown us, a boy who ran away from a neglectful father, was jailed after a friend framed him, and was rescued by Fugo and Bucciarati. Trish's situation is his own: a child the organization was going to discard. So the oath is not bravery, it is recognition. He is not choosing the Boss's enemies. He is choosing the one adult who ever chose him.
